Central Florida Collaborative 2018 - 2019 Community Health Survey

On behalf of AdventHealth, Aspire Health Partners, Orlando Health and the Florida Departments of Health in Lake, Orange, Osceola, and Seminole counties, we kindly request your participation in the Community Survey provided in the link below.
Our organizations are required to conduct a community health needs assessment (CHNA) every three years. In an effort to better serve the Central Florida community, we have again formed a collaboration to do a joint assessment, and the data collection for our 2019 assessment is currently underway.
The Community Survey aims to solicit your input to help identify the most important community health needs and issues specifically for the populations that you represent. We recognize that your unique perspectives will offer important insight into how to better serve our community together.
At the end of our CHNA, the information collected will be shared with all partners in the CHNA collaboration. This information will help us target gaps in services and better serve our area residents.
We ask that you complete this Community Survey by February 15, 2019. You can also share this survey with your clients, patients and other community partners
