Florida Department of Health in Seminole County

The Florida Department of Health in Seminole County provides children, families, men and women with quality, professional specialty care services. With special attention to preventative care and education, we assist with promoting the long-term health of our community and working together to ensure everyone has access to quality healthcare. Our primary office includes the following services making your visit easy to navigate. You do not need a referral to make an appointment, just call our office. When you call, we will collect information such as:

  • Your name, child’s name

  • Date of Birth

  • Phone numbers

  • Insurance information

What to Bring

  • photo ID

  • medical and pharmacy insurance cards, if eligible

  • preferred pharmacy name, location and phone number

  • names and dosage of all medications, including over the counter medication

  • guardianship and custody papers, if a legal guardian rather than a parent of child

Same day appointments are honored as schedule permits.  

Immunizations are on a walk-in basis until August 18 to accommodate back-to-school needs.
