In the past year, The Florida Department of Health in Seminole County’s Community Integrated Mobile Health Services (CIMHS) team provided over 2,500 services in Seminole County. In addition, CIMHS partnered with the Seminole County Fire Department to administer over 2,000 COVID-19 vaccines to homebound Seminole County residents.

The Community Integrated Mobile Health Services (CIMHS) is comprised a several programs such as the Mobile Health Unit, Pediatric Asthma, Cardiovascular Disease Prevention and Management and Community Paramedicine.

The Community Integrated Mobile Health Services (CIMHS) was established in December 2019 and comprises community outreach, home visit and post discharge follow up services to reach those who may have challenge with access to care. The Mobile Health Unit, Pediatric Asthma Home Visit Program and the Health Educator Post Discharge Follow up Program are included in this unit.

CIMHS is very versatile as it adapts to the ever-changing physical environment where patients are engaged, such as the patient’s home where we are mindful to aim at mitigating social determinants of health. Our comfort zone is the interface of the healthcare sector, patients care team, and patients’ home. CIMHS utilizes a robust community resource network to ensure that gaps in the patient’s care are filled while they seek to live a quality life managing chronic illnesses.
