Sanford, FL – The Florida Department of Health in Seminole County (DOH-Seminole) has established a new partnership with Unite Us, the nation’s leading software company enabling cross-sector collaboration to improve people’s health and well-being, through which

DOH-Seminole will leverage the Unite Us coordinated care network, known as Unite Florida, to securely send and receive referrals and track outcomes in a person-centered model.

“The partnership we have formed with Unite Us will allow us to expand existing connections with community resources and provide an accessible database for referrals helping improve health outcomes in Seminole County,” said Ana Scuteri, assistant health officer for the Florida Department of Health in Seminole County.

In Florida, Unite Us connects people to much-needed resources and services, such as assistance with food insecurity, housing, employment, and much more. Partners in the Unite Florida network are connected through the Unite Us shared technology platform, which enables them to securely send and receive electronic referrals, address people’s social needs, and improve health across communities.

Unite Us was launched at DOH-Seminole on April 17, 2023, and several community partners have already joined the platform, including Orlando Health, Recovery Connections of Central Florida, Seminole County Emergency Management, The Picnic Project, Hope Helps, and True Health.

In collaboration with hospital systems, community health systems, and healthcare agencies across the state, the partnership will connect the community support from DOH-Seminole with the reach and connection of the Unite Us platform. It will also combine the 2-1-1 community and information referral systems with best-in-class technology, enabling medical providers to connect patients to community resources that support better health outcomes through services and support.

This model has far-reaching impact for multiple reasons:

• The two organizations will combine their operation, technology, and financial resources to build a sustainable, common ecosystem that includes government, healthcare, and community sectors.
• Communities across Seminole County will be able to identify gaps in care and funding and address them through new partnerships and differentiated funding streams
• Community-based organizations will have access to top-notch technology tools to see the impact of their work and access real-time outcomes data.
