Community Integrated Mobile Health Services

The Fall Prevention Program is a no cost service offered by the Florida Department of Health in Seminole County to reduce incidents of older adults falls. Preventing falls in the elderly can significantly improve quality of life.


How to Prevent Falls

  1. Have your healthcare provider review your medication
  2. Exercise to improve your balance and strength
  3. Have your eyes, ears, and feet checked
  4. Make your home safer

Fall Risk Factors

  • Medication conditions
  • Medication use
  • Physical mobility
  • Vision impairments
  • Environmental hazards

Check your risk for falling by completing screening questions in initial screening form.


Home Visiting Program

Eligiblity
  • Age 65 and older
  • Seminole County resident
  • Score of 4 points or more on fall risk screening questions
Fall Prevention Home Visitting Program includes 3 home visits to:
  • Educate: Increase knowledge about fall prevention
  • Assess: Identify modifiable risk factors
  • Intervene: Use effective clinical and community strategies
