Community Integrated Mobile Health Services (CIMHS) facilitates community outreach, home visit, and post discharge follow-up services to reach those who may have difficulty accessing care. CIMHS are comprised of several programs:

The collective programs adapts to the ever-changing locations of our patients, including patients homes where we mitigate impacts to healthy outcomes. CIMHS utilizes a robust community resource network to fill the gaps in the patient’s care so they can live a quality life managing chronic illnesses.
