Reporting Information for Physicians, Laboratories, and Hospitals
If you are a health care professional or laboratory worker and would like to notify a reportable disease or outbreak during business hours, please use the practitioner disease report form for diseases other than HIV/AIDS, STDs, or tuberculosis, or contact the epidemiology department at 407-655-3243.
To report an urgent reportable disease or outbreak after hours, please contact 407-665-3000, option 1, and follow the instructions to reach the on-call epidemiologist.

Health care and laboratory providers are required to report conditions of public health importance to the Florida Department of Heath.
