PROGRAMS AND SERVICES Dental Care 407-665-3346 Provides services to children and limited urgent care for adults. Call for more information on services and payment options. Environmental Health 407-665-3604 Services include educating, regulating, and inspecting facilities and operations for cleanliness, safe food handling, and disease prevention. Immunizations 407-665-3281 Child and adult immunizations such as hepatitis A and B, Tdap, polio, HIB, HPV, chickenpox, measles, and meningitis. Immunization records, including Immunization DH 680 form. Travel Immunizations Provide vaccines for foreign travel and information about food and water conditions for disease prevention. Flu and Pneumonia Immunizations Flu vaccines are available seasonally; pneumonia vaccines are available throughout the year. Birth and Death Certificates 407-665-3226 Birth Certificates issued after 1930 for the state of Florida are available. Death certificates can be issued for deaths that occurred within a particular time frame. Diabetes Self-Management 407-665-3220 Provides adults with type 2 diabetes the skills necessary to manage their condition. Diabetes Prevention 407-665-3235 Trained lifestyle coach can provide tips and strategies to help participants prevent or delay the onset of diabetes Refugee Health Program 407-665-3243 Health assessments, immunizations, laboratory testing and referrals are provided to eligible clients. School Health Services 407-665-3433 Multi-faceted program serving public and private schools. Services include screening and monitoring of growth and development, dental, scoliosis, hearing and vision, coordination of school health services, and health education. Tobacco Prevention 407-665-3278 Provides education and training to prevent and reduce tobacco use and prevent exposure to second and third hand smoke. Specialty Care 407-665-3371 HIV/AIDS Services HIV/AIDS education, medical and case management services, and drug assistance program for qualified individuals PEP/NPEP Program HIV Post-Exposure Prophylaxis (PEP) is an antiretroviral medication taken 72 hours after a potential exposure to HIV to reduce the likelihood of HIV infection. Non-Occupational Post Exposure Prophylaxis (NPEP) is also available. PrEP Program 407-665-3690 Pre-Exposure Prophylaxis (PrEP) is a comprehensive HIV prevention strategy that involves the daily use of antiretroviral medications to reduce the risk of HIV infection in HIV-negative individuals. Tuberculosis (TB) Program 407-665-3243 Testing, diagnostic evaluations, treatment, and follow-up for individuals diagnosed with active TB disease and latent TB infection. Epidemiology 407-665-3243 Conducts surveillance and investigations of sporadic cases and outbreaks of reportable diseases and conditions. Afterhours Disease Reporting Line: 407-665-3000, Option 1 Hepatitis Prevention Program 407-665-3243 Provides prevention counseling, education, and testing for hepatitis A,B, and C Public Health Preparedness 407-665-3311 Responsible for coordinating Seminole County’s public health and medical preparedness and response to emergencies and disasters. This includes medical support to special needs shelters and dispensing of medications. Clinical Services Provides a wide range of services to improve the health and well-being of women, infants, children, and families. Breast and Cervical Cancer Program 407-665-3185 Offers free pap smears and mammograms for eligible women 50–64 years of age. School Physicals 407-665-3700 Physical examinations to meet the requirement for children enrolling in school. Family Planning Program 407-665-3700 Counseling and family planning options for men and women of child-bearing age. Sexually Transmitted Infections 407-665-3700 STI testing including confidential or anonymous HIV testing, prevention, diagnosis, treatment, and counseling. WIC Program 407-665-3705 Services for eligible pregnant and postpartum women, infants, and children under the age of 5. Nutrition education and counseling as well as breastfeeding support services are available. WIC services are offered in both Sanford and Casselberry. Insulin Distribution 407-665-3700 Must be a Seminole County resident. Financial Eligibility 407-665-3700 Patients eligible for the DOH-Seminole County Clinic Card will receive services at either a reduced rate or at no cost based on established guidelines.
